What is Health Coaching?

Working with a health coach, you will discover what your vision of optimal health and well-being looks like for you. Through your coaching partnership, you will learn ways to identify and navigate around the barriers between you and your ideal vision of your life!
THE CLIENT IS THE EXPERT
You are an expert of your body, mind, and circumstances! Clients know their bodies, their lives, and what’s worked well for them in the past. Health Coaches use a client-centered approach. Coaches are not experts on what their clients should do. Instead, they act as partners and facilitators, supporting their clients in acting, creating plans, and achieving goals based on the clients’ ideas, interests, and experiences.
INDIVIDUAL WELLNESS
It may seem hard or unclear where to start. For many, the knowledge, and instructions of what and how to start isn’t always enough to make the changes they desire. This is where a Health Coach can really play a critical role. Health Coaches can help you decipher the knowledge of what pertains to you individually and help you apply it.
BEHAVIOR MODIFICATION/ACCOUNTABILITY
Coaches also have the tools and skills to help clients build new habits and make lasting changes. This is what makes health coaches unique in the healthcare industry—they are not just a source of information but a catalyst for transformation.
HEALTH CARE PROVIDER TEAM APPROACH
Health Coaches work alongside your healthcare professionals in a complementary manner, following treatment plans prescribed by your individual medical provider.
What to expect in your Health Coaching sessions:
Initial Health Coaching Session
Together with your Health Coach you will:
Create: Ideal vision of your life.
Identify: Present situation, previous successes, challenges, and resources.
Establish Personal Goals: Identify attainable actions that need to be completed to create your ideal vision.
Decipher Your Individual Needs: Use the “I AM” Weight Loss Journey tool to select your unique needs.
Weight Loss Plan: Develop a series of steps that will lead to the achievement of your weight loss goals.
Establish Small Action Goal: To be completed before the next session.
Schedule Follow-Up Session: Health Coach will provide ongoing support and guidance as you set Weight Loss goals and make sustainable changes.
Health Coaching Follow-up Sessions
Together with your Health Coach you will:
Celebrate successes: Acknowledge accomplishments completed from previous session.
Reassess Small Action Goals: Make modifications based on personal decisions and progress made if needed.
Insights gained: Assist you in articulating learning from the change process.
Establish New Small Action Goal: To be completed before the next session.
Resources and tools: Health Coach will fill in the gaps where needed and provide direction to help attain goals.
Accountability: Health Coach will hold you accountable for established action goals.
Schedule Follow-Up Session: Health Coach will provide ongoing support and guidance as you set Weight Loss goals and make sustainable changes.
